South Manchester GP Federation / Manchester Primary Care Partnership South Neighbourhood meetings

South Manchester GP Federation / Manchester Primary Care Partnership invite you to their August Neighbourhood Partnership meetings (formerly called wider stakeholder meetings) for South Manchester.

The dates and times of the meetings are:

Didsbury, Chorlton and Burnage Neighbourhood Partnership – 2 August 2018 at 1.30pm

Withington and Fallowfield Neighbourhood Partnership – 9 August 2018 at 1.30pm

Wythenshawe and Northenden Neighbourhood Partnership – 16 August 2018 at 1.30pm

Wythenshawe Neighbourhood Partnership – 30 August 2018 at 1.30pm

Following on from the last meetings in June 2018 South Manchester GP Federation / Manchester Primary Care Partnership will be feeding back about the purpose of the partnership meetings moving forward as well as start to focus on more specific areas. 

One of the areas identified by Manchester Health and Care Commissioning (Manchester City Council and NHS Manchester CCG) and the Manchester Local Care Organisation (www.manchesterlco.org) is to improve outcomes for people at risk of and diagnosed with cardiovascular and mental health illnesses.

Known as Winning Hearts and Minds, this three year programme is a Community Development approach which recognises that while the direct causes of poor heart and mental health are clinical (blocked arteries, stress etc.) the causes of these are social and will require a wider community response to find solutions together.

It is known that smoking, drinking too much alcohol, poor diets and lack of exercise has a negative impact on health and wellbeing, but it is also known that social factors such as transport, housing, work and skills, community connections, the environment, having enough money and access to healthy food also influences health and decisions. Therefore everyone needs to work together across the whole system to make a lasting difference.

The focus of this first meeting will be to bring together a diverse group of people to talk about:
1. How do we identify and support people at risk in the community?
2. What can we do about it in this neighbourhood?
3. What is going well in the lives of people that have healthy hearts and minds?
4. How do we work together to support people with a diagnosis to manage their condition and improve their wellbeing?

Manchester has some of the worst health outcomes in the country, and South Manchester GP Federation / Manchester Primary Care Partnership would really value your contribution to these discussions.
