VCSE Home from Hospital services provision

As part of the Home from Hospital Programme, 10GM are currently mapping out the existing VCSE provision of hospital discharge and admissions avoidance support across Greater Manchester. This exercise aims to identify the needs of local communities with regards to hospital admission and discharge, and the gaps in current commissioning and provision.

10GM will be presenting a summary analysis of this mapping exercise at the January meeting of the Hospital Discharge Alliance on 25 January 2023.
