Services for employed people
These services are offered to individuals currently in employment who are off sick and are seeking to return to work.
For those in employment Pathways enables clients to access a biopsychosocial holistic service, which offers a guidance plus model, providing advice and guidance including employment law, communications with managers/peers, fast access to treatments including physio/psychological therapies, provision of culturally sensitive services, and support to deal with social issues including finances. The aim of the services is to help the individual move back into work quicker than without an intervention.
Services for unemployed people
For those not in employment Pathways enables clients to access a biopsychosocial holistic service, which offers a guidance plus model, providing advice and guidance including job-search, CV writing, job-keeping skills, job brokerage, employment law, fast access to treatments including physio/psychological therapies, provision of culturally sensitive services and support to deal with social issues including financial inclusion.
Health and Wellbeing:
This service supports people to improve their lives including managing their health condition e.g., those suffering from mild to moderate mental health issues (e.g., anxiety, depression), social isolation, poor motivation to self-care. Support provided includes:
o One-to-one and group support sessions
o Partnering with /signposting to other health and wellbeing providers
o Referral to other appropriate services
o Evidence based models delivered
Training and Consultancy
Pathways is an accredited Personalised Care Institute training provider through our Personalised Care Training Academy. We deliver personalised care training, to make personalised care business as usual. We develop evidence-based models that have the potential for wider take up and national impact. We produce guidance, model service specifications and training for commissioners and providers to refine early intervention / Primary Care initiatives to improve health and wellbeing through practical interventions for people whose health issues are affected by social factors in order to prevent the escalation of health issues and address health inequalities
