The commissioning strategy sets out South Manchester Clinical Commissioning Group’s vision and commissioning priorities for the coming years. It includes an outline of the South Manchester area in terms of population and need as well as explaining the financial picture in South Manchester.
The strategy proposes the following three key priorities for South Manchester Clinical Commissioning Group:
• Prevention - Improving health and increasing life expectancy by promoting and enabling self care, preventing ill health and providing fair and equitable access to local health and social care services
• Long-term and complex conditions - Early detection, prevention and management of people with long-term conditions and improving outcomes for people with mental health and learning disability problems
• Integration - To improve patient experience by working in partnership with patients and the local population of South Manchester and providing care close to home through the integration of health and social care services
The strategy sets out a number of objectives for each of these priorities and explores the local and national context, potential impact and the work South Manchester Clinical Commissioning Group will do to support each objective.
