Social prescribing and community-based support: Summary guide

NHS England has released a guide for people and organisations leading local implementation of social prescribing. It is designed to increase understanding of what good social prescribing looks like and how it can improve outcomes for people; to help with the commissioning of local social prescribing connector schemes; and to support collaborative working amongst all local partners at a ‘place-based’ local level. NHS England
