Do you want to be part of a new keyworker service ensuring children and young people with a Learning Disability and /or Autism, and their families have the right support, in the right way, at the right time?
This role requires passion and commitment, delivering quality support within the family home, placement or inpatient setting. You will work as part of a team to prevent where possible children and young people from requiring an inpatient admission or placement and/or to support a step down from this.
